Letter to the patients of Tina Siridakis ARNP

June 11, 2021 

Dear Patient, 

I am writing to inform you that effective immediately, I am formally resigning my position as Family Nurse Practitioner at Peninsula Family Medical Center (PFMC). I had hoped to be able to return to work after a long medical absence, but it became increasingly clear that I wouldn't be able to effectively perform my duties. It is with great regret and sadness that I make this announcement. 

I have so enjoyed caring for you during the past 3/2 years. I have always felt that being a health care provider has been my calling. You have enriched my life and learning and I am so thankful. 

I am very confident that you will be well cared for by any provider in this practice, whose practice is still open. It was one of the many reasons I chose to work at PFMC. 

Thank you again for the privilege of providing health care to you over the past few years. Take care and I wish you good health and happiness.
